Vision form upon submitting your package

Does the eye vision form have to be done upon submitting package, or can that be obtained further on down the road?

Reason I ask, I plan on dropping off my package next week, but can't get in to see my optometrist for a few weeks....

I think you should be able to drop off your package without the vision form but they will not process it until you have that form submitted. My advice would be to submit the personal history form and then the vision form as soon as possible.

yes... I agree about submitting in your package ASAP and then worry about your vision test...
